Marriage Forms - English
- Catholic to marry validly baptized Non-Catholic
  a copy of the non-catholic's baptismal certificate must be included
  
  Permission to Enter Mixed Religion Request (2 pages, please return both)
  
  Freedom to Marry Form (no previous marriages, first page only)
- Catholic to marry unbaptized/doubtfully baptized non-Catholic
  Dispensation from Disparity of Worship (2 pages, please print and return BOTH)
- Priest/Deacon to officiate marriage of Catholic/Non-Catholic at non-Church location (i.e. reception hall)
  Fill out top half of first page, then 2nd page, return both pages
  
  Permission for Priest/Deacon to witness ceremony outside of Church
- Non-Catholic Minister to officiate marriage of Catholic/Non-Catholic
  fill out first page, add parish address on 2nd page; return both pages
  
  Dispensation from Canonical Form (Non-Catholic Minister Officiating)
- Convalidation of civil marriage
  please keep this form in the records of the marriage; no need to send to chancery
  
  Convalidation Form

- All marriage dispensation requests are processed through the Office of the Vice Chancellor.
  
  All forms can be downloaded and copied for use in parish offices with the exception of the tri-fold PreNuptial Investigation form, which much be ordered from the Chancery.
